From 2016 to 2026, there were 518 incidents, with the majority of losses occurring in the last five years. Between 2021 and 2026, hackers stole approximately $15.2 billion across more than 450 incidents. In the past year alone, from April 2025 to April 2026, losses amounted to $2.5 billion over 140 incidents. Recent trends show a shift in attack methods, moving from exploiting smart contract vulnerabilities to targeting private key exposures and access control breaches.
